Notary Forms
Jurats
Jurats are best used for individuals signing or affirming that a written statement is true and valid. A Jurat is often employed when no other form, wording, or format is provided for your documentation.
CA Jurat with Affiant Statement
A Jurat with Affiant Statements are similar to normal Jurats. The only difference between the two is that Jurats with Affiant Statements have a blank space for the affiant to write there own statement whereas Jurats have a given statement attached.
Acknowledgements
The Acknowledgement form is used by an individual, who's identity has been verified by the Notary, as documentation to the Notary that the signer has willingly and truthfully signed the document.
Proof of Execution by Subscribing Witness
A Proof of Execution by Subscribing Witness is utilized when the primary signer cannot be present but directs a subscribing witness (third party) to testify to the primary party's affirmation and signature on a document.
CA Copy Certification of Power of Attorney
This Certificate is used to authorize documents copied from powers of attorney, as stated under Section 4037 of the California Probate Code.
